Developing these notes has taken me many hours, but once created they have proven invaluable.  Much to my surprise I have found that I no longer dictate any notes, these smart phrases are faster.

When you consider that each of these notes taken me several hours to write and debug the cost is a bargain.

Some of the functionality of these notes depend on using the EPIC problem list.  The recommended work flow is for the physician to review all the data, place orders, then update the problem list.  Once this is complete then use the smart note to create your note.  If after you create the note you think of additional information for the problem list the notes are refreshable, pulling in the updated problem list information.

By doing this on a daily basis each problem will have all the information needed to create a detailed discharge summary in just a couple minutes, with out dictating.

 

I recommend when documenting in the problem list to use the past tense and or date the data as you will not have to change it the next day or on discharge.   So rather then typing "the Hbg was 7.8", you would type " the Hbg on 9/15/11 was 7.8" rather than say "I will start the patient on empirical Rocephin and Zithro". You would say " On admission the patient was started on empirical Rocephin and Zithro".

To make these notes look even more professional I have a spot for the hospital logo.  It is a simple matter to copy and paste your logo into the space provided but I will do this for you if you like, you just need to email me your facilities logo or letter head.
